Carrier Requirements
  • Full name of company
  • Complete mailing and/or physical address
  • Phone, email, fax, mobile and contact phone numbers
  • Proof of liability coverage with a minimum of $1,000,000
  • Proof of cargo coverage with a minimum of $100,000
  • Operating authority with a M.C. number
  • W-9 form with a Federal Tax ID or Social Security Number
  • Rose City Transportation must be listed as a certificate holder on your insurance certificate
